Please complete the band information form and e-mail it back to us. This is the information we will use both in the program write-up of your band and during the announcement of your band as you prepare to perform.

Times are an example assuming a performance at 3:00 pm

Notes
- We will have water stations set up in the warm-up area and after exiting the field.
- We will have tractors and trailers to help you move your pit equipment to and from the field.
- We will NOT have anyplace available for your members to change before or after their performance.
- Ellsworth music will be on-site for any last minute equipment repairs or supply needs (drum heads, reeds, etc.)
